Join the Jean Tweed Centre as an Aftercare Counsellor, supporting women's recovery with evidence-informed groups and follow-up care in a hybrid role. Play a critical part in aiding clients transitioning from intensive treatment. This full-time, 18-month contract position emphasizes your expertise in substance use, mental health, and trauma-informed practices. You will facilitate aftercare groups such as SMART Recovery and DBT while connecting clients with essential community resources.
Your role involves tracking outcomes and providing follow-up support for up to six months post-discharge. Key Responsibilities:
- Facilitate weekly evidence-informed aftercare groups
- Offer follow-up calls and check-ins to clients
- Provide referrals and ensure integrated care
- Collaborate with social service agencies for client support
- Maintain documentation related to client interactions Requirements:
- Master's degree in Social Work, Psychology, or related field
- 3 years of experience in substance use and mental health
- Registration with relevant regulatory body
- Robust group facilitation and program development skills
