The Client Care Manager is a leadership role for a proactive, take-charge LPN or RN.

The Client Care Manager enjoys building strong relationships and creating a positive client and family experience. The role combines oversight of client care, quality standards, team collaboration, and community engagement to help ensure consistent, high-quality support throughout each client’s care journey.

Responsibilities

- Help maintain and ensure strong standards of care
- Manage and support day-to-day client care experience
- Oversee and strengthen the care assessment and quality assurance process
- Collaborate with the service team to support continuity of care
- Monitor service quality, client satisfaction, and team performance
- Maintain relationships with healthcare providers, senior living communities, and community partners




- Participate in community outreach and referral activities as needed
- Participate in the rotating after-hours escalation schedule

What We’re Looking For
- Current LPN or RN designation in good standing in Nova Scotia
- 5+ years' experience in senior care, home care, community healthcare, or a related setting

- Proactive, confident, and comfortable taking initiative
- Supervisory experience is an asset
- Strong communication, organization, and problem-solving skills
- Professional, compassionate approach
- Valid driver’s license and reliable vehicle
