At-Home Hospice is looking for an exceptional Registered Nurse or Registered Practical Nurse to join our team in a unique full-time hybrid clinical role combining community nursing with office-based clinical support.
We are not simply looking to fill a schedule. We are looking for a highly capable, dependable and experienced nurse who takes ownership, communicates exceptionally well, and can be trusted to make sound decisions when situations don't go according to plan.
For more than 20 years, At-Home Hospice has provided individualized nursing and personal support services to clients and families throughout the Ottawa community. Our care includes pediatric, adult, palliative and complex community nursing.
The Role
This is a full-time days and evenings position with a combination of community and office responsibilities. The successful candidate will work closely with our clinical and coordination team while also remaining actively involved in direct client care.
Responsibilities will include providing nursing care in clients' homes and community settings; completing client assessments and clinical documentation; supporting admissions, discharges and transitions of care; communicating with families, healthcare partners and community teams; assisting with care planning and clinical problem-solving; supporting caregivers in the field; participating in staff orientation, mentoring and education; and assisting the office team with clinical and operational needs as required.
The position also includes participation in a rotating after-hours on-call schedule. During assigned rotations, you will provide clinical and operational support to staff and families and assist with urgent scheduling or client-care concerns that arise outside regular office hours.
Who We're Looking For
We're looking for someone who stands out.
You are an RN or RPN in good standing with the College of Nurses of Ontario and bring strong clinical judgment, professionalism and confidence to your work.
You are comfortable working independently but also enjoy being part of a collaborative office and clinical team.
Most importantly, you are reliable. If you commit to something, your team knows it will be handled.
The ideal candidate is organized, calm under pressure, an excellent communicator and comfortable making decisions. Community, pediatric, palliative, complex-care or leadership experience would be a strong asset.
You should also be comfortable with technology and electronic documentation, possess a valid driver's licence and reliable transportation, and be willing to travel throughout the Ottawa area.
