Position Summary

Nodin’s Outpatient Mental Health Service (OMHS) assists persons who, under serious distress, have engaged in risky or unsafe behaviours due to an inability to cope as a result of immediate or historical factors. Using a multidisciplinary team approach to care, clients are matched with supports to best address their issues and needs and to enhance coping skills to deal more effectively with direct, intergenerational trauma and life’s difficulties. The interdisciplinary team provides case management, cultural support and teachings, mental health counselling, substance use counselling, safety planning, art therapy, and psychological care.

The Case Manager is a member of the OMHS multidisciplinary team who supports clients with practical, social, and service-related needs that affect their mental health and overall well-being. Working collaboratively with clients, families, community supports, and the interdisciplinary team, the Case Manager identifies barriers, coordinates services, facilitates referrals, and connects clients to appropriate community resources.

The Case

Manager assists clients in navigating health, social, educational, housing, financial, and other relevant systems, and participates in care planning, safety planning, and discharge planning to ensure case management interventions complement the clinical, cultural, and therapeutic supports provided by the OMHS team.

The Case Manager reports to the Outpatient Clinical Supervisor.

Annual/hourly Salary: $88,049 - $113,338 / $48.379 - $62.274

Qualifications

Education & Professional Credentials

- Minimum level of education required is completion of a diploma training program specific to mental health and social services from a recognized college.
- Current registration in good standing with a regulated professional body authorized to practice psychotherapy in Ontario (e.g., OCSWSSW, CRPO) with authorization for independent practice is required.
- Completion of additional training in cultural competency, trauma, suicide intervention, and family violence is considered an asset.

Experience & Clinical Knowledge





- Minimum of two (2) years of experience providing case management is required and/or equivalent combination of education and experience acceptable to SLFNHA.
- Experience supporting complex clinical presentations, including concurrent disorders, trauma, and high-risk behaviours.
- Solid understanding of theoretical frameworks underpinning mental health practice, including clinical assessment, case conceptualization, and treatment planning.
- Extensive knowledge of community resources and referral pathways or demonstrated willingness and ability to develop this knowledge.
- Ability to maintain high standards of ethical and professional practice.

Cultural Competence & Community Engagement

- Knowledge of the people, culture, and mental health priorities of First Nations communities in the Sioux Lookout region.
- Experience working with First Nations individuals, families, communities, and service providers is considered an asset.
- Ability to communicate in one or more First Nations dialects of the Sioux Lookout region is an asset.

Roles & Responsibilities Client Assessment, Care Planning & Case Coordination





- Complete comprehensive needs assessments and develop individualized service plans with clients and the multidisciplinary team, reflecting identified needs, strengths, goals, and priorities; provide plans and updates to referral sources as appropriate.
- Conduct regular check-ins to monitor progress, identify barriers, and revise service plans as needs change.
- Provide ongoing case management support, coordinating and referring clients to appropriate health, mental health, social, and community services, including First Nations, regional, and specialist supports.
- Advocate for clients navigating complex service systems, and maintain current knowledge of community and regional resources, eligibility requirements, and referral pathways.
- Develop and maintain relationships with First Nations communities and service providers to support coordinated care, effective aftercare, and clients' transition to community-based and regional services.
- Participate in discharge planning with the client, family, and treatment team; formalize discharge plans in writing, including recommendations, referrals, and follow-up actions.
- Communicate treatment summaries and follow-up plans to relevant providers, with appropriate consent, and follow up on referrals to support successful transitions from service.

Collaboration, Documentation & Professional Practice

- Participate in case conferences, supervision meetings, and treatment planning; collaborate with counsellors, cultural support workers, substance use counsellors, psychologists, and other team members to ensure coordinated, client-centered care.
- Maintain accurate, timely client documentation — including service plans, referrals, and discharge records — in the electronic health record system and in accordance with clinical standards.
- Contribute to the development of case management processes, referral pathways, and service delivery practices; deliver presentations and group work as requested.
- Maintain professional integrity, confidentiality, and appropriate boundaries; adhere to organizational policies and clinical standards.
- Perform other duties and responsibilities as assigned or required.
