Seniors Community Connector

Job Summary
We are seeking a dedicated Seniors Community Connector to bridge health and social care for older adults in Fort St. John and area. Using a person-centred, social-prescribing approach, the Connector will assess referred seniors’ strengths, needs, risks, and goals; develop individualized wellness plans; and connect them with non-medical supports through referrals, applications, advocacy, and community introductions. This role supports belonging, independence, healthy aging, and well-being.
Qualifications and Qualities
· Knowledge of issues affecting older adults—including healthy aging, frailty, social isolation, and access to community supports—or willingness to complete relevant training.
· Post-secondary education in Social Work, Human Services, Gerontology, Community Development, or a related field is preferred; equivalent education, training, and experience will be considered.
· Experience supporting older adults and people from diverse cultural, social, and socio-economic backgrounds is an asset.
· Knowledge of Fort St. John-area resources, service networks, programs, and supports for seniors.
· Strong collaboration and relationship-building skills with health-care professionals, community organizations, service providers, participants, and families.
· Strong communication, interpersonal, advocacy, public relations, organizational, research, coordination, and time-management skills.
· Ability to apply person-centred communication techniques, including active listening, motivational interviewing, and conflict resolution.
· Strong English communication skills and proficiency in Microsoft Office.
· Flexibility, sound judgment, and the ability to work independently and collaboratively.
· Valid driver’s licence and reliable personal transportation.
Key Duties and ResponsibilitiesCommunity Development and Partnerships
· Develop and maintain strong networks across the seniors-serving sector.
· Use an asset-based approach to engage people, organizations,
and service providers that support older adults.
· Engage community partners to expand support and improve seniors’ access to services.
· Lead and participate in communities of practice, planning groups, networking events, meetings, and shared learning opportunities.
· Coordinate regular seniors’ gatherings, dinners, and other assigned local activities.
· Work with health-care professionals, community agencies, and service providers to establish and maintain clear referral pathways.
· Collaborate with Seniors Community Connectors across British Columbia to coordinate community-based support for seniors at risk of frailty.
Referrals, Intake, and Participant Support
· Manage referrals and an active caseload, providing timely follow-up based on participant needs, risks, and available supports.
· Prioritize referrals based on individual needs and program capacity.
· Complete intake and assessments to identify each senior’s strengths, abilities, needs, risks, routines, safety concerns, supports, and goals.
· Use appropriate assessment tools and person-centred techniques, including active listening, motivational interviewing, conflict resolution, and behavioural observation.
· Follow all safety protocols during community meetings and home visits.
· Protect the confidentiality of participant, family, partner, and organizational information.
Wellness Planning
· Develop individualized wellness plans with seniors based on their goals, preferences, strengths, risks, and support needs.
· Use a “what matters to you” approach to connect participants with suitable community services, programs, activities, and resources.
· Assist with referrals, applications, advocacy, introductions,
and other practical connections to non-medical supports.
· Track participant engagement and update wellness plans as needs, circumstances, or goals change.
· Inform seniors and families about local resources and, when appropriate, connect them with primary care, seniors’ services, and multidisciplinary supports.
· Complete training in healthy aging and social prescribing.
Documentation and Program Support
· Record participant interactions, assessments, wellness plans, referrals, follow-ups, reports, and required administrative information.
· Track program statistics, including contacts and calls, and maintain a daily activity journal.
· Support program evaluation by collecting participant data, completing reports, and contributing to ongoing improvement.
· Use reflective practice and participate in relevant communities of practice.
· Attend required staff meetings, in-service workshops, and job-related meetings.
· Provide vacation or operational coverage as needed and perform other assigned duties.
Position Details
· Location: Fort St. John, BC
· Employment type: Permanent part-time, subject to funding
· Start date: As soon as possible
· Term: Until March 31, 2028, subject to probation and funding, with continuation intended if funding is available
· Pay: $24.00–$28.00 per hour, based on experience
· Transportation: Applicants must be able to commute reliably to Fort St. John or relocate before starting
