The Grand Bend Area Community Health Centre (GBACHC) is seeking a 1.0 FTE family physician to join our dynamic, interprofessional primary care team. Located along Ontario's beautiful Bluewater coast, the GBACHC provides comprehensive, team-based care to diverse populations across Grand Bend, Hensall, and surrounding communities. Role Overview

As a family physician at the GBACHC, you will work in a collaborative model alongside Nurse Practitioners, Registered Practical Nurses, dietitians, social workers, physiotherapists, Occupational Therapists, health promoters, and other inter-health professionals. Physicians at the GBACHC play a key role in:

Providing comprehensive, evidence-based and holistic primary healthcare, following the Health Disciplines Act of Ontario, the College of Physicians and Surgeons of Ontario (CPSO) practice standards, and Community Health Centre (CHC) guidelines

Supporting chronic disease management, preventative care, and health promotion initiatives

Participating in interprofessional case conferencing and quality improvement initiatives

Contributing to the teaching and mentorship of learners when applicable.

Qualifications

Independent practice license with the CPSO

Certification in Family Medicine (CCFP) from the College of Family Physicians of Canada (CFPC) or equivalent experience is an asset

Strong commitment to interprofessional, team-based primary care

Experience and/or interest in community health, chronic disease management, and preventative care





Demonstrated commitment to equity, diversity, and inclusion in healthcare A completed Vulnerable Sector Police Check completed within the last six months
