This permanent, full time role requires strong leadership in program integration and service quality improvement. Reporting to the VP of Integrated Care and Experience, you will manage diverse teams to deliver equitable and accessible health services.

Your responsibilities will include overseeing integrated programs and ensuring exceptional service delivery while fostering collaboration among staff. Your engagement will reflect the values of the Durham Community Health Centre and focus on reducing barriers to care. Key Responsibilities:

- Support operational plans aligned with DCHC's strategic priorities
- Supervise and develop interprofessional staff for optimal service delivery




- Champion quality improvement and manage change initiatives
- Develop community relationships for program effectiveness
- Prepare performance reports and identify improvement opportunities Requirements:
- Undergraduate degree in relevant field; Master’s preferred
- At least 5 years in health or social service management
- Proven experience in integrated care and team management
- Familiarity with population health and equity frameworks
- Strong leadership and communication skills Lead the transformation of integrated care ensuring equitable service access for all communities.
