About Us The Athens District Family Health Team is located in beautiful Eastern Ontario, between the historic Rideau Waterway and the world-famous Thousand Islands. We are a small, supportive, and genuinely close-knit team that values collaboration, respect, and looking out for one another.

Our multidisciplinary team includes physicians, nurse practitioners, nurses, a health educator, and dedicated administrative staff. We believe that a positive workplace culture is just as important as excellent patient care.

The Opportunity

We are seeking a Registered Nurse to join our team in a flexible capacity. We are open to casual, part-time, or full-time arrangements depending on the successful candidate's availability and career goals.

This position may be particularly appealing to a nurse who currently works part-time or casual in another healthcare setting and is looking to gain experience in primary care while maintaining flexibility. It is an excellent opportunity to get your foot in the door in family medicine and become part of a collaborative healthcare team.

For candidates interested in full-time work, we anticipate a temporary six-month contract focused on supporting physicians with patient assessments, chronic disease management, preventative care initiatives, and administrative clinical support.

What You'll Do

- Work collaboratively with physicians and other team members to provide high-quality patient care.
- Assist with patient assessments, follow-up care,



and monitoring of ongoing health concerns.
- Support preventative care and health promotion initiatives.
- Provide patient education and counselling as appropriate.
- Assist with chronic disease management programs and care coordination.
- Support physicians with clinical documentation, patient communications, referrals, and other administrative activities.
- Contribute to a positive, respectful, and team-oriented workplace culture.

Who We're Looking For

- Current registration in good standing with the College of Nurses of Ontario.
- Registered Nurse (RN) designation - or - RPN with primary care experience.
- Strong assessment and clinical reasoning skills.
- Excellent communication and interpersonal skills.
- Commitment to patient-centered care.
- Ability to work independently and as part of an interdisciplinary team.
- Primary care experience is an asset but not required.
- New graduates are encouraged to apply.
