Responsibilities

- Assess patient conditions and develop personalized care plans aligned with medical protocols and patient needs.
- Administer medications, injections, and treatments accurately following established procedures.
- Monitor vital signs, perform phlebotomy procedures, and assist with laboratory testing to support diagnosis and treatment.
- Administer Methadone and Suboxone dose to patients and keeping record and documentation
- Collaborate with physicians, case managers, and other healthcare professionals to coordinate comprehensive patient care.
- Document all patient interactions thoroughly using electronic health record systems such as Epic or Cerner.




- Educate patients and families on health management strategies, medication usage, and post-discharge care instructions.

Skills

- Proven experience working in hospital environments with acute care expertise.
- Familiarity with electronic health record systems like Epic and Cerner for efficient documentation.
- Strong knowledge of medical terminology, medication administration protocols, and laboratory procedures including phlebotomy.
- Experience in methadone and suboxone dispensing.
- Ability to manage casework effectively across diverse patient populations including assisted living and nursing home residents.
- Excellent communication skills to collaborate seamlessly with multidisciplinary teams and educate patients effectively.
- Demonstrated competence in pain management techniques and medical office procedures related to nursing practice.
