Key Responsibilities: • Conduct and document thorough client assessments • Develop and communicate care plans with the healthcare team • Identify health education needs and provide guidance • Order and interpret diagnostic tests as needed • Facilitate referrals to specialists and health services
Requirements: • Must be a registered Nurse with Ontario's College of Nurses • 2+ years of experience with complex medical issues • Previous NP experience in outpatient settings is an asset • Proven ability to work independently • Bilingual English and French proficiency required
