What would my role be?

Reporting to the Director, Family Health Services, you will:

Work collaboratively with members of the health care team in providing healthcare for family medicine patients

Provide specialized care to an identified group of patients who are at risk

Participate in committees which address the needs of at-risk patients

Provide education to learners and staff.

Required :

Undergraduate degree in Nursing, Master's degree for graduates after 2011

Valid certificate of registration from the College of Nurses of Ontario (CNO) as NP-PHC

Current membership in Nurse Practitioners' Association of Ontario (NPAO)

Primary Health Care Nurse Practitioner Certificate.

Qualified Liability insurance with Canadian Nurses Protective Society

Valid Basic Life Support (BLS) cardiopulmonary resuscitation (CPR) Level C certificate

Recent related clinical experience (work or educational related)

Preferred :

Fluent in French and English

What skills should I possess to succeed in the role?

Excellent patient assessment, critical thinking, clinical reasoning, communication (oral and written), and problem-solving skills.





Practice autonomously according to the NP standards of practice

Able to work in an inter-professional team environment.

Able to prioritize, manage time effectively and be flexible in a very active work environment.

Intermediate computer skills, including in electronic health records

Advanced knowledge of medical terminology
