The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care.

Family Health

Team staff includes nurse practitioners, registered nurses, social workers, pharmacists, dietitians, occupational therapists and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centred care. Academic preparation from an accredited Canadian university program in foods & nutrition or equivalent Practical training in an accredited practical program in Canada or equivalent Three to five years clinical counseling and community nutrition experience or comparable skills, experience and education Experience in program development, implementation, monitoring and evaluation Member in good standing with College of Dietitians Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery,



with an emphasis on health promotion and risk reduction Experience in the use of EMR and Microsoft Office applications Multi-tasking skills related to the delivery of efficient primary care Solid understanding and experience with adult group learning principles, teaching self management techniques & ability to evoke participation in a group environment Level of comfort or willingness to become familiar with virtual meeting technology Experience with facilitation of an online program considered an asset Experience and commitment to working collaboratively with other healthcare professionals Experience in working with patients with a history of trauma is an asset, and experience in trauma informed care.

Hourly Rate: $34.8138 - $42.4559 (dependent on experience) To be eligible to apply you must have a Social Insurance Number and in some cases a valid work permit.
