Liaise with clients, families, referral sources, funders, and internal teams to support continuity of care. You have a Bachelor of Social Work or a Master of Social Work. ~ You are a registered member in good standing with the Alberta College of Social Workers. ~1 to 2 years of community experience is an asset. ~ You have ability to travel within the Hawkesbury community as part of the CBI Health Home Health Services team.
