LOFT Community Services is seeking a Case Manager, Behavioural Support to join the Behavioural Support Services Team in Aurora, ON. You will deliver direct care and coordinate a behaviour support network for seniors with dementia, mental health, addictions or neurological conditions, including transition planning and collaboration with families and healthcare teams.

Successful candidates have post-secondary education in Social/Human Services, at least 3 years’ experience with the client
