Vancouver Coastal Health in British Columbia is seeking a Community Health Nurse to deliver home-based care within an interdisciplinary team.

You will assess clients, develop and implement care plans, provide direct nursing care, and coordinate supports to promote independence in community settings. You will collaborate with clients, families and other providers to ensure secure transitions across services, and document activities in the EMR.
