Join LAMP Community Health Centre as a Community Health Worker focused on preventative care for Black, African and Caribbean communities. This role emphasizes culturally responsive interventions in Peel Region and requires proactive outreach efforts. We are looking for a dedicated Community Health Worker for the Black Health and Social Services Hub.

Your role will involve planning and delivering health initiatives that address the unique needs of BAC communities. You will support clients with needs screening and referrals, while conducting outreach and collaboration with local organizations. Key Responsibilities:

- Plan and deliver community group programming
- Conduct needs screening and support health referrals
- Engage in outreach across the Region of Peel
- Collaborate with the BHSS Hub team on resources
- Maintain accurate client records in the EMR Requirements:
- 2+ years in community healthcare
- Diploma or degree in relevant fields
- Experience with Black, African and Caribbean communities
- Skills in group facilitation and outreach
- Ability to manage competing priorities Drive community health improvements through culturally informed care for BAC communities in Peel.
