Extended Length of Stay (ELOS) Risk Assessor Extended Length of Stay (ELOS) Risk Assessor - Hospital to Home Permanent Full time Shift Assignment Site Liase with Agency staff for programming. ~Meet with patients to complete screening tools used to assess risk for an extended length of stay. ~Determine priority of focus in order to achieve an estimated date of discharge (i.e. clinical, social, functional). ~Liaise with Ontario Health at Home, Community Support Services agencies and the Service Provider Organization in patients’ onboarding, discharge and ongoing programming. ~Act as a point of contact for departmental staff, other departments, programs, and community stakeholders as required. ~Provide a link between the hospital community agencies and alternate level of care (ALC) facilities. ~Communicate across care team and engage with community partners effectively. ~Provide support for patients, caregivers and informal supporters being served by the organization and community agencies. ~Determine and align improvement projects with HSN’s Strategic Plan; Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required. ~Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines. ~EDUCATION AND TRAINING: ~Minimum of a Bachelor’s Degree in health related field or equivalent, from an accredited university.



~Training in Lean methodologies is preferred. ~Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

Experience with Home First Implementation is preferred. ~Experience working with data management software is preferred. Demonstrated knowledge of risk assessment tools (i.e. Demonstrated knowledge of health research and/or best practice guidelines in discharge planning andALC Avoidance. ~Demonstrated knowledge of the Transitions Between Hospital and Home Quality Standards. ~Demonstrated knowledge of Medical Terminology. ~Demonstrated knowledge of current trends in general health care and community resources. ~Demonstrated training, experience or utilization of quality improvement methodology for processimprovement. ~Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g.

Word, Excel, Power Point and Outlook) and patient information systems. ~Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workloadand rapidly changing priorities and challenging deadlines. ~PERSONAL SUITABILITY: ~Demonstrated ability to perform with minimal supervision; Ability to travel between local sites.
