Become a pivotal part of the Mobile Response Team in Brockville, ON, as a Registered Practical Nurse. You'll provide direct care and support for older adults with responsive behaviours in a long-term care home setting. As a full-time Registered Practical Nurse at Providence Care, you will collaborate with an inter-professional team of RN/RPNs and PSWs to enhance the quality of life for your clients.
Your role will involve hands-on support and interventions for older adults experiencing dementia, mental health issues, and other neurological conditions. Join a supportive work culture committed to person-centred care practices and quality improvement initiatives. Key Responsibilities:
- Provide direct care and support to clients in collaboration with the MRT team
- Assist in developing Collaborative Care Plans tailored to client needs
- Conduct assessments using evidence-based tools
- Support the MRT PSWs through assignments and collaboration
- Work across multiple long-term care homes within your region Requirements:
- Current unrestricted RPN registration in Ontario
- Minimum one year of relevant experience in elder care
- CPR and First Aid certificates required
- Experience with responsive behaviours in various care settings
