Extendicare's Columbia Forest home in Waterloo, Ontario seeks a dedicated MDS/RAI Co-ordinator to lead timely assessments, integrate InterRAI tools with care plans, and train staff. You will analyze data for quality indicators and liaise with Health Zone/CIHI to ensure accurate reporting.

The ideal candidate has 3–5 years in long-term care with MDS experience, robust analytical skills, and the ability to educate adult learners.
