Niagara Medical Group Family Health Team is an interdisciplinary primary health care team in Niagara Falls, On. Located on Queen Street, just down from City Hall. We are seeking a permanent full-time dedicated Nurse Practitioner to join our team.

The ideal candidate will have a passion for working with diverse populations and providing clinical and therapeutic services. This position is in partnership with the Youth Wellness Hub Niagara, located in Welland, ON. The position will be part-time in the Family Health Team and part-time at the Youth Wellness Hub.

Position Requirements

- Perform patient health assessment and health maintenance activities of all ages with a focus on 12 – 25 years of age
- Monitor the ongoing therapy of patients with chronic illness, including palliative and mental health illness
- Diagnose and treat acute/episodic minor illnesses
- Integration of psychotherapeutic approaches to care
- Experience with supporting diverse communities
- Collaborate with Community Partners, families, caregivers
- Offer phone and home consultation/triage for acute and episodic illnesses
- Consistently record and document in the patient’s electronic medical record




- Determine the need for and order all necessary diagnostic imaging, medications, treatments and referrals
- Prepare and report quarterly data for Ontario Health
- Knowledge and experience of disordered eating, gender-affirming care an asset

Education, Credentials & Experience

- Completion of college/university/CEGEP/vocational or technical training
- Master’s degree in nursing and/or equivalent graduate Degree
- Current Registration in good standing with the College of Nurses of Ontario as a Registered Nurse, Extended Class with Nurse Practitioner (Adult)
- One to two years’ clinical experience as a Nurse Practitioner
- Clinical experience in Mental Health an asset
- Basic computer knowledge and competence (word processing/clinical applications)
- Electronic Medical Record knowledge is an asset
- CPR/Basic First Aid Certification
