Bridges Community Health Centre (CHC) is a non-profit, community-governed organization funded by the Province of Ontario. Our holistic, interdisciplinary model of health and wellbeing focuses on primary health care, health promotion, illness prevention, and community capacity building, while considering underlying social determinants of health and barriers to accessing care. Our greatest strength is our team, who is committed to the work they do, serving and strengthening the communities of Fort Erie, Port Colborne and Wainfleet.
We are currently recruiting for two permanent, full-time Registered Practical Nurses to join our team.
The Registered Practical
Nurse works in collaboration with a multi-disciplinary team in health promotion and health prevention. Current registration and in good standing as a Registered Practical Nurse (RPN) with the College of Nurses of Ontario Diploma in Practical Nursing from a recognized college. Minimum three years’ experience as a RPN in a community setting (primary care office setting an asset) with demonstrated experience working within an interdisciplinary team.
Proficiency in Electronic Medical Records and virtual care tools. Ability to practice in a client-centered approach with a strong understanding of the impact of the social determinants of health. Ability to be flexible and adaptable to an evolving environment.
Adherence to Bridges CHC’s policies and procedures. Upholds our Model of Health and Wellbeing, Mission, and Vision. Bilingualism an asset. Ability to work other hours as needed by the organization, including evenings and weekends.
Visiting clients in the community as needed, with access to a reliable vehicle with a valid Ontario driver’s license. Working to the full scope of practice of an RPN as per the College of Nurses of Ontario Triage client phone calls. Provide advice based on assessment, or determine the appropriate treatment or service, and the appropriate care provider.
Call clients with results and recommendations from RN, NP and MDs. Primary care new client intake coordination, as per applicable triaging and processes. Maintain accurate and timely EMR documentation as per CNO and CHC standards.
Collect and chart reportable data. Adhere to infection prevention control policies (IPAC) which may include instrument reprocessing. Administer and document immunizations and injections. Perform wound care and suture/staple removal.
