Vancouver Coastal Health (VCH) is seeking a Primary Care Nurse for the UBC Urgent and Primary Care Centre team in Vancouver, BC. The nurse triages, assesses, plans, and evaluates nursing care for high-priority patients within 24 hours and collaborates with primary care providers in the local Community of Care network.
The role applies chronic disease management, palliative, and self-management principles to care delivery, within the framework of BC nursing standards and VCH policies.
