Portage Medical Family Health Team is an interdisciplinary primary health care team in Niagara Falls. We are seeking a dedicated Nurse Practitioner to join our team. The ideal candidate will have a passion for working with diverse populations and providing clinical and therapeutic services.
Responsibilities
- Perform patient health assessment and health maintenance activities
- Support with Primary Care appointments
- Support our Woman's Health Programming
- Monitor the ongoing therapy of patents with chronic illness, including palliative and mental health illness
- Diagnose and treat acute/episodic minor illnesses
- Offer phone and home consultation/triag for acute and episodic illnesses
- Consistently record and document in the patient's electronic medical record
- Determine the need for and order all necessary diagnostic imaging, medications, treatments and referrals
- Prepare and report quarterly data for Ontario Health
Education, Credentials & Experience
- Completion of College/University/CEGEP/Vocational or Technical training
- Masters degree in nursing and/or equivalent graduate Degree
- Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse, Extended Class with Nurse Practitioner (Adult)
- 1-2 years clinical experience as a Nurse Practitioner
