Effectuer les évaluations des soins des résidents, dresser les plans de soins et de services, concevoir d’autres documents et s’assurer qu’ils sont exacts, à jour et que les changements sont communiqués, au besoin, à la directrice des soins pour la facturation. Contribuer à la planification des réunions sur les soins des résidents et y participer. Travailler en étroite collaboration avec la directrice des soins pour faciliter l’emménagement, le déménagement et le transfert des résidents.

Contribuer à l’éducation des résidents et des familles. Superviser les évaluations et vérifications de l’automédication des résidents. Superviser les fournisseurs de soins non réglementés et leur déléguer les responsabilités en matière de soins et d’administration des médicaments conformément aux règlements applicables et aux politiques et procédures d’Extendicare.

Planifier, coordonner, gérer et évaluer les plans de soins des résidents en collaborant avec les résidents ou leur mandataire, les médecins, le personnel infirmier, les membres de l’équipe interdisciplinaire et les fournisseurs de services contractuels. Coordonner les affectations du personnel infirmier aux soins des résidents pour assurer la prestation de services efficiente et en temps opportun. Réagir aux changements importants dans l’état de santé des résidents ou aux urgences médicales et les gérer.

Assurer la surveillance et la coordination des mesures d’urgence pendant l’absence de la directrice des soins et de la directrice générale. Donner suite aux préoccupations des résidents et des familles et voir à ce que les mesures appropriées soient prises ou que la préoccupation soit communiquée à la directrice des soins ou à la directrice générale et à d’autres services. Aider la directrice des soins à coordonner, à mettre en œuvre et à surveiller le programme de prévention et de contrôle des infections, le programme de santé et de sécurité, les programmes cliniques et les plans de qualité pour le service des soins infirmiers.

Coordonner et superviser les horaires et les affectations des étudiants en soins infirmiers d’établissements d’enseignement approuvés. To complete and ensure residents’ care assessments, service plans and documentation are up to date, and changes communicated to DOC for billings. To assist with scheduling of residents’ care conferences and participate in conferences.

To work closely with DOC to facilitate residents’ move in and transfers.



To order and safe keep drugs and narcotics and administer in compliance with all regulatory requirements and Extendicare policies and procedures. To facilitate and provide resident/family education.

To supervise resident self-medication assessment and audits. To supervise and delegate care and medication administration responsibilities to Unregulated Care Providers in compliance with applicable regulations and Extendicare policy and procedures. To plan, coordinate, manage and evaluate the resident’s care plan working with the resident/substitute decision maker, physicians, nursing and interdisciplinary team and contracted service providers.

To coordinate nursing team’s resident care assignments to ensure efficient and timely delivery of services to residents. To respond to and manage significant changes in residents’ condition and or medical emergencies. To act as supervisor and coordinate emergency responses in absence of DOC and Executive Director.

To respond to residents/family concerns and ensure appropriate action taken and or concern communicated to DOC/Executive Director and other departments. To assist the DOC in the coordination, implementation and monitoring of Infection Prevention and Control, Health and Safety and Clinical Programs and Quality Plans for the Nursing Department. To coordinate and supervise the scheduling and assignment of nursing students from approved education institutions.

Current registration as a Registered Nurse in applicable Province/State Experience working with seniors and unregulated care providers Education and experience in gerontology, rehabilitation, community/complex/long term care and assets. Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills. Must have demonstrated sound clinical knowledge and ability to delegate to unregulated care providers.
