Southbridge Roseview in Thunder Bay, Ontario is seeking a Social Worker to provide emotional, social, and practical support to residents and families. You will promote well-being, address psychosocial needs, and act as liaison between residents, families, staff, and community resources.
The role supports two homes on a biweekly basis, reports to the Executive Director, and leads care conferences, education, and documentation in accordance with the Long-Term Care Homes Act.
