Niagara Medical Group Family Health Team is an interdisciplinary primary health care team in Niagara Falls, On. Located on Queen Street, just down from City Hall. We are seeking a full time permanent dedicated System Navigator-Finance to join our team. The ideal candidate will have a passion for working with diverse populations and providing education and assistance navigating complex health, social and community support systems.

Position Responsibilites

- Investigating health inequity and the relationship with social determinants of Health
- Providing Financial Literacy and Budgeting education to clients
- Income Tax Returns
- Preparation and presentation of workshops for the community
- Assessing social needs of patients using screening tools (in areas including nutrition, housing, transportation, etc.)
- Understanding and prioritizing challenges faced by patients
- Tracking patient responses and compiling data to make reportable conclusions
- Providing follow-up sessions to assess effectiveness of navigation and program referrals




- Prepare and report quarterly data for Ontario Health

- Demonstrate leadership abilities

Education, Credentials & Experience

- Completion of 2 – 4 year college/university/CEGEP/vocational or technical training
- Completion of Accounting, Business (Finance) Degree
- Basic computer knowledge and competence (word processing/clinical applications)
- CPR/Basic First Aid Certification
- Electronic Medical Record knowledge is an asset
- Clinical Experience an asset

Salary: $75,950 – $77,673 annually
