Enhance quality of life through VHA Home HealthCare as a Visiting Nurse specializing in Palliative Care in Durham Region. Collaborate with healthcare teams to provide holistic end-of-life care. VHA Home HealthCare is seeking Registered Nurses (RNs) and Registered Practical Nurses (RPNs) to support our dedicated Palliative Care program.

You will assess, monitor, and coordinate nursing services while working collaboratively with external professional teams. This role requires a valid Ontario driver's license and access to a reliable vehicle to travel between client locations. Key Responsibilities:

- Assess and monitor client needs and conditions
- Evaluate and adjust care plans regularly
- Ensure quality and safety in patient care delivery
- Document client interactions and maintain standards
- Participate in on-call rotations as necessary Requirements:
- Registered with the College of Nurses of Ontario
- Degree/diploma in nursing from a recognized institution
- CPR or First Aid Certification
- Knowledge of wound care and community nursing
