Senior Support Coordinator – Homewood Health CentreMake a difference in a growing, innovative mental health organization and centre of excellence in people‑centered care.What You’ll Be DoingProvide support coordination for evidence‑based, complexities within the senior population with serious mental health and concurrent disordersConduct comprehensive patient assessmentsProvide support coordination within a recovery framework to include treatment and community supports with both formal and informal networksEnsure clients are supported in community environments, with engagement in community locations at a minimum of 75% of the service periodWhat We’re Looking ForBachelor’s in Psychology, Social Work or other related fieldValid registration with the relevant college/governing body in OntarioKnowledge of older adult population needs and concurrent mental health disorders1 – 3 years’ experience in concurrent mental health and knowledge of the complexities within the senior populationValid driver’s license, business insurance and personal vehicleHours of WorkPermanent Full Time, 35 hours per weekAs part of our commitment to the health and safety of our patients,
