This part-time role focuses on delivering person-centred support, using your knowledge to guide clients through various pathways. Your interactions will include face-to-face meetings, telephone calls, and virtual support to empower individuals in their community, ultimately fostering independence and reducing isolation.
Key Responsibilities: • Connect clients to essential local services and groups • Provide one-on-one guidance tailored to individual needs • Conduct assessments and create personalized support plans • Use various communication methods to reach clients • Foster emotional support and enhance mental well-being
Requirements: • NVQ Level 2 in Health and Social Care or working towards it • Proven experience in assessing client needs • Solid alignment with Making Space values • Excellent communication skills • Ability to work collaboratively in the community
