As an Enhanced Care Nursing Coordinator, you will assess, plan, implement, and evaluate care for enhanced care clients.
Your responsibilities involve collaborating with interdisciplinary teams and community health providers, ensuring client comfort and wellness. This role requires a comprehensive understanding of mental health support, wound care, and effective communication skills. Key Responsibilities:
Assess and plan care for enhanced care clients
Provide treatment services for mental health and addiction
Supervise care aides and client interactions
Maintain accurate communication and case notes
Ensure compliance with policies and reporting obligations Requirements:
Active RN registration with BCCNM
Wound care experience required
Class 5 Driver's License and reliable transportation
Robust communication and interpersonal skills
