Now Hiring: Assistant Care Manager

What You’ll Do

We are seeking a dedicated and organized Assistant Care Manager to support our care management team in providing exceptional care for residents in a long-term care setting. This role involves assisting with care planning, coordinating with multidisciplinary teams, and ensuring that residents’ needs are met efficiently and compassionately.

· Assist the Care Manager in developing, implementing, and reviewing individualized care plans for residents.

· Coordinate with nurses, therapists, and other healthcare specialists to ensure continuity of care.

· Monitor residents’ health, progress, and well-being, reporting any concerns promptly.

· Maintain accurate and up-to-date documentation in accordance with company and regulatory standards.

· Support residents and their families by providing information, guidance, and assistance as needed.

· Participate in team meetings, training sessions,



and quality improvement initiatives.

· Provide support during meetings with union representatives and assist with union-related matters.

What You Bring

· Previous experience in a healthcare, long-term care, or care management role preferred.

· Strong organizational and communication skills.

· Ability to work collaboratively in a multidisciplinary team.

· Compassionate, patient-centered approach to care.

· Knowledge of care planning, documentation, and healthcare regulations is a plus.

· Relevant certifications or education in nursing, social work, or healthcare management is desirable.

· Must be a Health Care Assistant (HCA) with a valid certificate in good standing.
