Job Description AMANI: Mental Health & Substance Use Services for Black Youth ( www.amaniservices.ca ) is Canada’s first hospital-community partnership of its kind, focused on advancing care for Black children, youth and families. This model of care provides safe, accessible, Africentric and racial trauma-informed support for Black youth (between 12 and 29 years old) and families/caregivers who are impacted by mental health and/or substance use health concerns across ten (10) locations throughout Ontario.
AMANI is seeking a full time, permanent Nurse Practitioner who will report to the Senior Manager of Child, Youth and Family Services and work collaboratively with other clinical services within the CYEAP and across CAMH, including working collaboratively with CAMH’s Black Health Strategy team. The Nurse Practitioner will work within the scope of practice of a Registered Nurse (Extended Class) [RN(EC)], and will practice within AMANI’s interprofessional Consultation-Liaison (CL) Service and Virtual Care Team (VCT), supporting Black youth (and families/caregivers as appropriate) between the ages of 12 and 29 years old, who are navigating mental health and/or substance use health concerns.
Responsibilities You will also provide consultation to other professionals working with this population across CAMH, and consult and collaborate with the VCT on a weekly basis to support clients in navigating to appropriate hospital- and community-based care providers and/or resources as needed, including close collaboration with AMANI Community Partner Sites. The successful candidate will also be responsible for monitoring, triaging and processing referrals,



coordinating and facilitating various clinical/educational group sessions on an as-needed basis, and documenting participant progress.
Centrally located within AMANI, the service supports continuity of care through a range of clinical and psychosocial interventions. Services include medication management, housing and financial support, and psychosocial support. The successful candidate will work collaboratively within the interprofessional team and liaise with teams across other services. You will provide comprehensive care to clients including: conducting health assessments; providing follow-up and treatment; offering expert consultation to clients and members of the interprofessional team; planning, implementing, and evaluating direct care; providing clinical leadership related to the delivery of care for patients and their families; and working autonomously and accountably in communicating diagnoses, prescribing medications, and ordering diagnostic tests. The Nurse Practitioner will lead and manage crisis responses, including suicide risk assessments, demonstrating knowledge of the Mental Health Act. The successful candidate will practice in accordance with the College of Nurses of Ontario practice standards, the Canadian Nurses’ Association Advanced Practice Nursing Framework, and all applicable legislative requirements.
