The numbers — from actual billing data, not projections
In our experience, our general full-time clinic cardiologists averaged:
Qualified collections (physician revenue) C$1.26 million
Overhead - 20%, capped at C$10,000 + HST/month C$135,600 all-in (≈11% effective)
Net physician income C$1.13 million (≈US $800,000)
Production (CMS wRVU-equivalents, professional work) ≈ 18,000 wRVUs

Taxes and cost of living — the part everyone asks about
Practice through your own professional corporation: retained earnings are taxed at ~11–12%, a deferral vehicle with no U.S. employed-physician analogue. U.S. citizens: cross‑border tax structuring matters (CFC/GILTI rules) — we include specialist cross-border advice in the recruitment package.
Figures are from actual OHIP payments to two current full-time clinic cardiologists over the most recent full year, at fee levels that have since increased (April 2026 schedule); individual results depend on clinical mix and volume. wRVU-equivalents are computed by mapping each Ontario fee code to the 2025 CMS work-RVU of its closest CPT equivalent; methodology available on request.
