Make a difference as a Registered Practical Nurse with Ontario Shores Centre for Mental Health Sciences in the Hospital to Home Program. Provide compassionate, recovery-focused care to patients facing complex mental illnesses.

This role reports to the Clinical Manager for Complex Services and focuses on delivering specialized support for clients with severe and persistent mental health issues. You will engage with multidisciplinary teams, clients, and community partners to facilitate successful recoveries through effective discharge planning.

Key Responsibilities:



• Deliver specialized care in line with recovery-focused principles • Collaborate with multidisciplinary team members and community partners • Support patients transitioning from hospital to home • Maintain accurate documentation and patient records • Model the Recovery philosophy in all care aspects

Requirements: • Registration with the College of Nurses of Ontario • Current CPR certification for Health Care Providers • 5 years psychiatric experience preferred • Excellent communication and interpersonal skills • Valid G class Driver’s License and attendance record
