Key Responsibilities:
• Provide personalized care and support to a roster of clients
• Conduct joint care planning with interdisciplinary teams
• Address clients’ physical, cognitive, and emotional domains




• Collaborate with primary care clinicians to achieve care goals
• Ensure continuity of care during home transitions

Requirements:
• Valid Certificate as a Personal Support Worker (PSW)
• RN or RPN qualification preferred
• Foreign trained healthcare professionals are acceptable
• Valid driver’s license and vehicle for work
• Current vulnerable sector check (VSC)
