Bridges Community Health Centre is seeking two permanent RPNs to support clients in Fort Erie, Port Colborne, and Wainfleet. The ideal candidate will have a diploma in Practical Nursing, current registration with the College of Nurses of Ontario, and three years of experience in a community setting. Familiarity with Electronic Medical Records and client-centered care are key attributes for success in this role.

Key Responsibilities:
• Work within the full scope of RPN practice
• Triage client calls and arrange necessary appointments




• Coordinate new client intakes and document results
• Maintain accurate EMR documentation and data collection
• Administer immunizations and perform wound care

Requirements:
• Current registration with the College of Nurses of Ontario
• Minimum three years' community nursing experience
• Proficiency in electronic health records
• Strong communication and problem-solving skills
• Valid Ontario driver's license for community visits
