In this role, you will deliver comprehensive assessments, interventions, and health education tailored to complex chronic conditions. Working with an interprofessional team, you will facilitate care plans and help clients transition smoothly across the healthcare system. Key Responsibilities:
Build therapeutic relationships with clients
Assess health needs and coordinate care plans

Current practice as a Licensed Practical Nurse in BC
One year of relevant direct care experience
