Northern Health seeks a Care Process Coach to join our Primary Health Care department. The Coach provides mentorship, coaching, and change management for primary care teams, guiding development and implementation of interprofessional processes aligned with local patient needs.
Using LEAN and evidence-based quality-improvement methods, the incumbent designs, implements, and assesses recent processes, workflows, and tools to enhance care delivery, safety, and staff experience across the local system.
