Trillium Health Partners in Mississauga is seeking a Part time Care Navigator for THP@home to help discharge patients safely to home with wrap‑around supports, reducing unnecessary hospital stays.
Reporting to the Clinical Manager in Primary and Integrated Care, the successful candidate will assess patients, arrange transportation, coordinate with community and primary care providers, and ensure follow-up calls and effective care planning.
