Responsibilities include managing a varied caseload, executing skilled nursing interventions, and collaborating with other healthcare professionals. You will travel within designated geographic areas to meet client needs effectively.

Key Responsibilities:
• Conduct comprehensive nursing assessments and develop care plans
• Manage a community caseload with proactive communication
• Provide skilled nursing care including wound and medication management




• Educate clients and families on treatments and symptom management
• Maintain accurate electronic documentation and manage visit schedules

Requirements:
• Active RN registration with the College of Nurses of Ontario
• Current CPR and First Aid certifications
• Valid driver's license and access to a vehicle
• Solid critical-thinking and clinical assessment skills
• Experience with community healthcare is an asset
