Join CMHA Toronto as a Case Manager in the Patterson House Program, supporting individuals transitioning from homelessness to stable housing. Make a difference in mental health service delivery through client-centered engagement and advocacy.
As a Case Manager, you will report to the Program Manager, assisting clients in accessing transitional and supportive housing. This role requires at least three years of experience working with adults facing responsible mental health challenges. Strong assessment and crisis prevention skills are crucial, as you will engage directly with clients and liaise with community partners.
Key Responsibilities:
• Conduct outreach and assessments for clients
• Facilitate client orientation to programs and services
• Provide personalized interventions and support
• Develop clients' skills and set achievable goals
• Maintain strong relationships with clients and landlords
Requirements:
• Bachelor's or diploma in Social Work/Social Science
• Minimum 3 years' experience with complex mental health issues
• Knowledge of supportive housing and recovery principles
• Strong communication and interpersonal skills
• Valid Ontario driver’s license and access to a vehicle
