The Service Planning Coordinator provides services in a manner consistent with Developmental Services ofLeeds and Grenville Mission, Vision and Values and that reflectthe core values of Client and Family Centred Care works as a member of theChildren’s Services Team at DSLG and in conjunction with KidsInclusive tosupport families of children and youth with multiple and/or complex special needs by acting as theidentifiable point of contact and being responsible for developing a multipleagency coordinated service plan that recognizes all of their service needs andbuilds on their child/youth’s strengths; helps to connect families with theright services in their service delivery area and monitors how children andyouth are progressing through the service plan as they grow; provides directand indirect client care; assists in providing department support; documentshis/her involvement in client care; maintains communication with all otherstaff members; supports departmental efforts in Continuous Quality Improvementactivities; participates in education orientation and public relations, asrequired; maintains a safe work environment; plays a lead role in the Communityof Practice for Coordinating Service Planning.
The position of the Service Planning Coordinator Isresponsible for determining a client’s eligibility and support needs throughcompletion of the applicant referral process (as identified through the LLGSpecial Needs Steering Committee). The position also provides counseling,direct services and supports for individual clients, their families andsignificant others. These services may include parent/mediator trainingassistance and support, basic counseling and service coordination. The purposeof these services is to determine the support required for clients and familiesin accessing services and resources to maximize an individual client’spotential. Upon the establishment of a support plan a program is developed tomap the individual’s potential and provide family support as a whole.
MINIMUM QUALIFICATIONS
4 year Bachelors Degree (University degree/undergraduate degree) in the Humanities, Psychology, Sociology or a related field
3 years related experience
Extensive work experience with families of children with complex special needs
Knowledge of the needs and best practice service standards for children with developmental disabilities
Able to work with families from various socio-economic and cultural backgrounds
Able to work with multi-disciplinary team members
Able to work with people in crisis
Good interpersonal skills including the ability to build rapport and trust
Excellent oral and written communication skills
Strong analytical skills
Knowledge of municipal, provincial and federal government resources and other community programs and services
CPI, CPR and First aid training




Bilingualism preferable and considered a strong asset
Valid Ontario driver’s license and access to a reliable vehicle
DUTIES AND RESPONSIBILITIES
1. PROVIDES INTAKE SERVICES BY:
Completing the applicant referral process todetermine eligibility for services
Gathering historical and other pertinent informationnecessary for the completion of the individual’s developmental history
Using the information to develop a support plan
Prioritizing service needs and identifyingactivities that will promote independence and allow the individual to maximizetheir potential
Referring persons who do not meet DSLG’s mandate toother community agencies
Liaising with multiple agencies to develop a clientcentered plan
Developing a single plan of care between agencies
2. PROVIDES SERVICES TO CLIENTS BY:
Developing case plans reflecting goals andmethodologies that are consistent with the strengths and support needs outlinedin the Children’s Support Plan
Providing service coordination and advocacy servicesas required by accessing, liaising, coordinating inter- and intra-agencyservices (i.e. case planning meetings, IPRC meetings, treatment team meetingsetc)
Providing parent training and/or mediation trainingin the areas of child development and child behavior management
Adapting information as required and ensuringappropriate supports are available to families to carry out goal plans
Assisting families to respond to issues related tohousing, employment, finances, education, etc
Assisting families to complete funding applications
Ensuring client contracts are complete andup-to-date
Identifying gaps in service and, where appropriate,creating specialized programming (i.e. social skills training, parental skillstraining etc)
Completing annual case plans summarizing servicecommitments and associated costs
Providing information and strategies to clients andmediators on the facilitation of adaptive skills
Accessing assessment services and assisting familiesto implement recommendations through support and mentoring
3. RELATED TO POLICIES AND PROCEDURES:
Operates in accordance with applicable Legislative,Ministry and professional association requirements of the position
Is knowledgeable about, ensures others areknowledgeable about and complies with Quality Assurance Measures (QAM)guidelines as directed by MCCSS
Is knowledgeable about, ensures others areknowledgeable about and complies with the agency’s policies and procedures
Operates, at all times,



in a safe and securemanner
5. HEALTH AND SAFETY:
Ensure Health and Safety Program requirements arefollowed based on the Occupational Health and Safety Act
Ensure an awareness of safety measures for the agency,yourself and the individuals receiving services
Ensure compliance with the agency’s mandatorytraining requirements

About Us Since 1983 DevelopmentalServices of Leeds and Grenville (DSLG) is a non-profit, charitable organizationproviding services and supports for people with developmental disabilities.Incorporated in May 1983, DSLG began operations in July of that year under thename Brockville and Area Centre for Developmentally Handicapped Persons Inc.(B.A.C.D.H.P. Inc.). DSLG offers a variety of clinical and family support services to children and adults living in the counties of Leeds and Grenville. Services are available to individuals who are experiencing a developmental/ intellectual disability or in some cases to children who may be at risk for a developmental/intellectual disability. Services are also available to the family, the school, the community and other human service organizations that affect the individual. We strive to make our broad range of support services inclusive, individualized and emphasize full participation in community life. We work collaboratively with our community partners to be innovative and translate our fundamental beliefs into daily practice.
Referrals for services can be made by any individual or agency, providing consent has been obtained from the individual or family, by calling the Brockville office.
