What would my role be?
Reporting to theDirector, Family Health Services,you will beresponsiblefor the measurement and reporting of quality assurance indicators, asidentifiedby the Director of Family Health Services and any other internal or third-party reporting.
Yourmain responsibilities include:
Contributing to evaluating programs, processes and health outcomes and providing information and recommendations to the practiceswith regard tobest practices with integrated care delivery
Working with the Director, Family HealthServicesand medical leadership to establish and maintain the communication and team building mechanisms across community partners and clinics
Assisting with special projects and other duties as required
Preparing, coordinating and managing all communication and documentation for Committees chaired by the Director of Family Health Services
Required:
Three (3) yearUndergraduatedegree in business administration, health care, or other relevant field, or equivalent combination of experience, education, and training
Minimum of three (3) years’ experiencein managing projects, deliverables,
or client relations,in program development,stakeholder engagement, and quality improvement
Proven working experience as a Data Analyst or Business Data Analyst
Preferred:
Post-secondary education in clinical, health science, information technologyand/or health informatics
Registered member of a regulated health professional college with health informatics experience
Fluent in French and English

Knowledge and competence in addressing the continuous quality improvement process
Excellent organizational, project, and time management skills and ability to manage multiple projects and priorities in limited time and to completion
Demonstrated experience in program development, including providing coaching and guidance to staff
Highly developed interpersonal and communication skills and the ability to work effectively with a wide range of people including senior leaders, physicians, external partners and agencies
Critical thinker and problem solver, with an ability to develop solutions to complex problems
Superior writing skills including report writing
Ability to multi-task, problem solve, and apply good judgement and decision-making across areas of program management, workflow processes, communications, and operational accountabilities
Flexibility, adaptability and innovation in practice in a continuously changing environment
Meticulous attention to detail
Excellent knowledge of various software programs such as Microsoft Office 365, Adobe,
and other computer-based programs
Functioning of an Electronic Medical Record
Technical expertise regarding data models, database design development, data mining, and segmentation techniques
Advanced knowledge of Medical Terminology
