Support Indigenous communities as a Wellness Provider with the VNFC in Victoria, BC. This role focuses on coordinating health care access and delivering culturally safe support.
As an Indigenous Wellness Provider, you will be part of the interdisciplinary VNFC Wellness Team. Collaborating with clients and local Indigenous communities, you will develop holistic care plans and assist those facing various health barriers. You will provide educational support, coordinate access to resources, and facilitate community health events throughout the Victoria and Western Primary Care Networks (PCNs).
Key Responsibilities:
• Coordinate client-centered health and wellness care plans
• Provide transportation support for medical appointments
• Organize community health activities and events
• Educate health providers on cultural safety
• Collaborate with local agencies to enhance service utilization
Requirements:
• Degree or diploma in Social Work, Human Services, or related field
• Experience in community social services and knowledge of Indigenous issues
• Valid BC Class 5 Driver’s license
• First Aid certification
• Transparent vulnerable sector criminal record check
