In this vital position, you will work Monday to Friday during day shifts. Your role includes performing intake assessments and crisis interventions for patients facing mental health and substance use challenges. Additionally, you will adapt care plans and contribute to patient flow coordination within the Emergency Department and with community providers.

Key Responsibilities:
• Conduct intake assessments for mental health and substance use patients
• Offer crisis intervention and support across care settings




• Collaborate effectively with Emergency Department and nursing staff
• Assist in ensuring smooth patient flow during assessments
• Adapt care plans to meet individual patient needs

Requirements:
• Experience in mental health or substance use care
• Solid assessment and crisis intervention skills
• Proven ability to work in interdisciplinary teams
• Knowledge of community resources and services
• Commitment to patient-centered care
