What you will do
Deliver high-quality community-based physiotherapy interventions within patients’ homes.
Work collaboratively within an integrated health and social care service alongside Occupational Therapists and support staff.
Apply your rehabilitation and orthopaedic experience to support patient recovery and maximise independence.
Assess, recommend, and provide minor equipment to support daily living and mobility needs.
Conduct holistic assessments to identify and address wider health and wellbeing needs.
Complete falls risk assessments and implement appropriate prevention and management plans.
Develop and review personalised treatment plans to achieve positive patient outcomes.
Maintain accurate clinical records and documentation in line with professional standards.
Promote independence, functional mobility, and quality of life through evidence-based interventions.
Requirements
HCPC Registration
Current registration as a Physiotherapist with the Health and Care Professions Council.
Driving Licence
Full UK driving licence essential for community visits.
Community Experience
Previous experience within community physiotherapy services is desirable.
Rehabilitation Expertise
Experience in rehabilitation and orthopaedic physiotherapy.
Assessment Skills
Strong clinical assessment and treatment planning abilities.
Communication Skills
Excellent interpersonal skills with the ability to work effectively with patients, families, and multidisciplinary teams.
