The SPPCN is seeking a dedicated and compassionate Registered Nurse (0.6 FTE) for a unique and rewarding position in our Seniors Clinic . You will work with a multidisciplinary team to provide comprehensive, patient-centered care for older adults, supporting healthy aging, cognitive screening, chronic disease management, and proactive care planning.

This is an excellent prospect to practice full scope, apply clinical expertise, and make a meaningful difference in the lives of seniors within the community. Monday, Thursday, Friday Assessment of patient conditions, needs and goals in collaboration with the patient, attending physician and interdisciplinary team Provide short term interventions as needed Facilitate appropriate referrals to community resources and programs Participate in evaluation of the clinic and its services Assist in planning, developing, and monitoring for services provided Assist in the development of policies and protocols for chronic disease management, geriatric care,



maternal/child and mental health Assist in setting up health promotion programs relevant to the needs of the practice population Assist in implementing strategies for improving the quality and provision of care for the practice population Baccalaureate degree in Nursing preferred Five years or more of related nursing experience Experience in PAP testing Experience working in a team with other health professionals Knowledge of nursing processes and practices including intervention strategies and therapy Ability to build working relationships with healthcare professionals, clients and community members Customer service orientation; Basic computer skills and knowledge of electronic medical records Personal Attributes and Competencies Flexible, creative, and able to juggle competing priorities Satisfactory Criminal Records Check with Vulnerable Sector Valid driver's licence and access to a vehicle for travel to stakeholder locations Flexibility in hours with potential early mornings and evening hours;
