Become a pivotal part of the Canadian Mental Health Association as a Transitional Case Manager in Oxford, Ontario. This full time contract role emphasizes supporting clients with mental health and addiction needs in achieving recovery goals. In this full-time, 12-month contract position, you will collaborate with individuals experiencing mental health challenges.

Your core responsibilities will involve client assessment, goal setting, and implementation of individualized rehabilitation plans. This role offers a chance to positively impact lives through client-focused support and the development of coping strategies. Key Responsibilities:

- Conduct various client assessments for needs and barriers
- Assist clients in setting and achieving personal goals




- Develop and implement individualized rehabilitation plans
- Provide education on mental health and available resources
- Coordinate crisis management and support services Requirements:
- Diploma or degree in health or social services discipline
- Valid G class driver’s license and access to a vehicle
- Satisfactory Police Vulnerable Sector Check
- Ability to regularly attend work
- Experience with serious mental illnesses preferred Make a difference in the lives of those facing mental health challenges in Oxford.
