Your role involves improving patient outcomes through comprehensive pharmacotherapy assessments, active communication with patients, and supporting operational activities. You will interact closely with several rehab units, providing drug information services and participating in education and training initiatives. Key Responsibilities:

- Provide patient-focused care to enhance patient outcomes
- Conduct thorough pharmacotherapy assessments and medication reconciliations




- Facilitate drug distribution and procurement processes
- Train and mentor recent pharmacy staff and learners
- Communicate effectively with healthcare teams Requirements:
- Bachelor's or Doctor of Pharmacy degree required
- Active Ontario pharmacy registration and competency certificate
- Hospital pharmacy experience preferred
- Excellent communication and organizational skills
