This full-time role requires strong management skills, clinical assessment expertise, and a commitment to ethical care. You will collaborate with nursing partners and engage patients in identifying their health needs while ensuring compliance with standards and policies. Key Responsibilities:

- Supervise PSWs and HSWs, ensuring adherence to practices
- Resolve community staff and patient concerns effectively
- Lead regular team meetings and one-on-ones
- Initiate patient safety investigations and documentation
- Collaborate on clinical documentation per funder guidelines

Requirements:

- Current RN or RPN registration in Ontario
- Minimum 5 years of nursing experience
- Solid patient health assessment skills
- Proven experience in team leadership
- Valid driver’s license and reliable transportation
